Information you give us. When you request a consultation online, our form (provided by Jotform under a HIPAA business associate agreement) asks for details such as your name, phone number, email, preferred office and preferred times. When a dental office refers a patient online, our referral form (also provided by Jotform under a HIPAA business associate agreement) asks for the referring office’s details, the patient’s contact information and date of birth, the reason for referral, and any X-rays or records the office chooses to upload. When you call us, we receive your phone number.

Information collected automatically. Our hosting provider records basic technical data (such as IP address, browser type and pages requested) to deliver and secure the site. We do not use website analytics, and we do not use cookies to track visitors.

• To receive and act on referrals from dental offices, and to keep the referring office informed.
• To keep the site secure and meet legal obligations.

We do not use advertising pixels, tag managers or analytics on this website. Our consultation and referral forms run on Jotform’s HIPAA-compliant service inside a secure frame: what you type goes directly to Jotform, not to this website, and it is handled as protected health information under HIPAA.

We share information only with service providers that help us run the site and the practice (for example website hosting with Vercel and the form provider, Jotform), when the law requires it, or with your permission. We do not sell personal information, and we do not share it for targeted advertising.

This website does not set advertising or analytics cookies. The embedded Jotform forms may set cookies that are needed for the form to work. You can block or delete cookies in your browser’s settings; you can always call our office instead of using the online forms.

This website is not intended for children to submit information. A parent or guardian should request appointments for patients under 18.

We use reasonable administrative, technical and physical safeguards, including encrypted connections (HTTPS). We keep website information only as long as needed for the purposes above or as required by law.

You may ask to access, correct or delete personal information we hold about you from this website, subject to the law and to records we must keep. Rights over your health records are described in our Notice of Privacy Practices. To make a request, contact us as shown below.
